What is home health care and who is it for?

Many times, it’s for an aging family member who needs help with everyday tasks and wants to remain at home. But it can also be for someone of any age who is recovering from surgery or an injury, requiring rehabilitation, or living with a long-term illness. In addition to general care, we also specialize in helping clients who have a Traumatic Brain Injury (TBI), Alzheimer’s and other types of dementia, developmental disabilities, or who have ventilators and tracheotomies.

Every client receives a plan of care customized to his or her exact needs. Home care can be daily visits, round-the-clock nursing, live in help, or occasional visits throughout the week. It can be short-term or long-term and can adapt over time to life’s changes.

Just some of the many services our team members can provide:

Nutritious, comforting meals – cooked and served

Medication reminders

A warm bath or shower with a waiting towel

Dressing, personal hygiene, and toileting assistance

Support with walking or using a wheelchair

Accompaniment to doctor appointments

Help with day-to-day tasks such as shopping, cooking, light housekeeping, and laundry

Skilled nursing and/or rehabilitative care for recovery from illness or injury

Temporary help to relieve family members for short periods of time

Our Caregivers

Every caregiver on the All Metro team has been carefully screened to make certain he or she meets our high standards for care delivery. In addition, each member of the team is overseen by a Registered Nurse (RN) who will visit each home on a regular basis to ensure the health and safety of our clients, and make certain that the client’s needs are being met by the care team.

Types of Caregivers:

  • Home Health Aides or Personal Care Aides – Assistants with some Medical Training
  • Companions/Homemakers – Non-Medical Assistants
  • Skilled Nurses – Registered Nurses and Licensed Practical Nurses
  • Rehabilitation Professionals – Physical, Occupational, and Speech Therapists

For clients who prefer to hire their own caregiver, we can assist with that as well. Under the Consumer Directed Personal Assistance Program (CDPAP), New Yorkers may have the option to recruit, hire, train, and supervise individuals to provide them with personal assistance services. All Metro is an authorized Fiscal Intermediary for this program and we provide this service through most Medicaid Managed Care plans.

Programs that Cover the Costs of Care

In many instances home care can be covered completely or partially under different financial assistance programs. All services are also offered on a private pay basis. All Metro will help you find and access the program that best fits your needs and that provides the maximum benefit.

Medicaid Managed Care or Medicaid Managed Long Term Care

States offer a range of Medicaid home care service programs, each oriented toward slightly different needs and sometimes to different populations. Often, benefits may only be accessed through Medicaid Managed Care Plans and Medicaid Long Term Care Plans. All Metro works closely with almost all Managed Care Plans inour markets.

Nursing Home Transition and Diversion (NHTD) Medicaid Waiver

This Medicaid program is available to certain New York residents. Through this program, Medicaid funding provides additional supports and services to seniors and individuals with disabilities who choose to remain in or return to their community from a care facility.

Traumatic Brain Injury (TBI) Medicaid Waiver Program

This Medicaid program is available to certain New York residents who have suffered a traumatic brain injury (including acquired brain injuries) and choose to live at home, rather than in a nursing care facility.

Care for People With Intellectual and Developmental Disabilities

There are many Medicaid programs designed to help people with intellectual and developmental disabilities thrive in the community. All Metro participates with several of them as part of our commitment to ensure our clients receive the high-quality services necessary to live a full and healthy life.

Long Term Care Insurance

Many people have Long Term Care insurance to cover some or all of the expenses related to home care, from assistance with simple activities including bathing, dressing, and eating, to more skilled care provided by nurses and therapists. All Metro has contracts with most Long Term Careinsurance companies.

Additional Programs

There are numerous other programs available to our clients to help pay for the services All Metro provides. These include programs offered by the Veterans Administration and the Area Office on Aging in your county, Workers Compensation insurance, automobile insurance and many others. Our team is here to help you determine the best path to coverage available.
