Live-In Home Care
It is the goal of the company’s Live-In Home Care Program to provide experienced caregivers and a range of supervised, professional services that meet the patient’s home care needs under one predetermined fee.
All Metro Health Care’s Live In Home Care Program takes into account the health and home support needs of the patient while providing consistent, personal attention on a one on one basis. The individual and family gain the following benefits with around the clock services from All Metro Health Care: (i) live at home with all personal care ensured; (ii) maximum privacy and confidentiality; (iii) expertise of a trusted live-in caregiver; (iv) one on one service; (v) supervision by a Registered Nurse; (vi) minimal disruption to normal life; (vii) respite, relief and reassurance for the whole family; (viii) comfort and companionship; and (ix) affordable alternative to hourly rates.
Before services commence, one of All Metro Health Care’s Registered Nurses, in consultation with the patient’s attending physician and family members, will visit the patient’s home or facility, from which the patient is being discharged, to develop a comprehensive plan of care. In conjunction with family members, who may be active participants in the patient’s care, our primary goal is to help the patient attain his or her highest level of well-being.
After services begin, periodic home supervisory visits are conducted by Registered Nurses to assure the services are being provided according to the plan of care. Furthermore, written daily patient progress notes are made available to the patient’s physician, authorized family members, and case managers. In the event that an issue or question arises, the company’s office staff and nursing supervisors are available 24 hours a day, 7 days a week, 365 days a year.
